Who is this survey for?
This survey is for people living with chronic obstructive pulmonary disease (COPD) in Prince Edward Island. If you are completing this survey on behalf of someone living with COPD, please answer all questions (for example: their age, gender, etc.) based on the individual living with COPD.

Why should I complete the survey?
Health PEI wants to better understand your experience of living with COPD, what helps you as you manage your COPD every day, and the challenges you experience.
What will Health PEI do with the findings of the survey?
The information gathered through this survey will be used by Health PEI to identify needs to guide future planning of COPD programs and services.
How long will the survey take to complete?
The survey should take no more than 20 minutes to complete. All information provided will be kept confidential and anonymous.
