Diabetes Needs Assessment Survey 

Who is this survey for?

This survey is for people living with diabetes in Prince Edward Island regardless of their type of diabetes    (Type 1 diabetes, Type 2 diabetes, Pre-diabetes). Caregivers, including parents of children living with diabetes are also invited to participate. If you are completing this survey on behalf of someone living with diabetes, please answer all questions (for example:  their age, type of diabetes, etc.) based on the individual living with diabetes.  

Is there a deadline to complete the survey?

The deadline to complete the survey is July 31, 2018.

Why should I complete the survey? 

Health PEI wants to better understand your experience of living with diabetes, what helps you as you manage your diabetes every day, and the challenges you experience. 

What will Health PEI do with the findings of the survey? 

The information gathered through this survey will be used by Health PEI to identify needs to guide future planning of diabetes programs and services. 

How long will the survey take to complete?

The survey should take no more than 20 minutes to complete.  All information provided will be kept confidential and anonymous.

How do I complete the survey?

The survey can be completed online.
